Our team at Recovery Epicenter Foundation understands that certain events and circumstances in the past, may have have put the need to take care of our physical health on the back seat. We have compiled a directory of local medical and dental providers that provide a multitude of healthcare services for free and on a sliding scale.
This directory is composed of 2 sections. The top of the page contains a list of Medical and Dental providers, and the bottom of the page contains links to Insurance providers for the indigent.

To be eligible, you must:
- Be a resident of mid or upper Pinellas County (parents must provide documentation of address)
- Be a US citizen or lawful permanent resident
- Meet low-income financial guidelines
- Not have any insurance, including Medicaid, County Social Services, KidCare
Overview of services:
Patients have access to a full range of medical services that are provided on-site within our primary care setting. Other services, including diagnostic procedures, are provided by community partners.
Comprehensive care includes:
- Primary Care and Specialty Care Office Visits: EKGs, labs, and x-rays, Hospital procedures (as warranted)
- Women’s Care: Pap smears and mammograms
- Diabetic Program: Care, nutrition and education
- Medication Service (NO NARCOTICS): Acute and chronic medications, Patient Assistance Program, enrollment
- Ophthalmology: Diagnostic eye exams (when available), Diabetic eye exams
- Nutritional Counseling: One-on-one counseling and group classes
- Pediatrics: Sick visits, Back to school physicals
- Behavioral Health: Referral only from Clinic practitioner
In addition: Physical Therapy, Ear, Nose and Throat Program, Patient Education Program & Coumadin Clinic.

Upon arrival, the following process takes place:
- IMMEDIATE NEEDS MET: Shelter, food and clothing provided.
- 12-24 HOURS – REFERRED TO DENTAL & WELLNESS: Upon intake: Clients are referred to HEP Dental & Wellness Clinic for a checkup and/or to receive treatment.
- 24-72 HOURS – ASSESSED MENTALLY & PHYSICALLY: An on-site medical and mental health assessment is performed through partnerships with Morton Plant Mease Outreach and Baycare Health Systems through the HEP Dental & Wellness Clinic.
- WITHIN 3 DAYS – TREATMENT PLAN ESTABLISHED: 1-on-1 meeting with Case Manager to determine a customized treatment plan.
- WITHIN 30 DAYS – ACCESS TO SUPPORT SERVICES: Client gains access to all supportive services including: child care, food stamps, Medicaid, transportation, benefits assistance, and more.
- PROGRESSION TOWARDS INDEPENDENCE: Client continues to access supportive services for duration of stay. Weekly 1-on-1 meetings with Case Manager ensure progress toward set goals.
- EMPOWERMENT – INDEPENDENT HOUSING ACQUIRED: Upon exit, client is self-sufficient enough to acquire independent housing, or may be referred to another agency better suited for their unique individual needs.

Sliding Scale
This clinic operates under a SLIDING SCALE model. This means that it MAY NOT be free depending on an individual’s income. Individuals will be required to prove financial need in order to receive free services or services at a reduced cost. This is a health care center funded by the federal government. This means even if an individual has no insurance, he/she can be covered. The center is also income based for those making an income. This health center can cover services such as checkups, treatment, pregnancy care (where applicable), immunizations and childcare (where applicable), prescription medicine and mental and substance abuse where applicable. The Homeless Dental Clinic provides multiple layers of care. Those layers range from medical care to housing. Their purpose is to break the cycle of homelessness and to be the last homeless shelter an individual or family ever has to access. They monitor their programs to measure progress against stated goals and make necessary refinements throughout the process. Since 1986, their mission has been to provide homeless and low-income individuals and families, including veterans, with housing, food, clothing and support services necessary to obtain self-sufficiency and improved quality of life.

The Community Dental Clinic provides basic dental services (hygiene, composites, extractions) to the uninsured adult community in upper Pinellas County. Patients are qualified based on their income and their location of residence.
To meet eligibility requirements of the Community Dental Clinic, patients must fill out intake paperwork. Appointments are offered every Wednesday from 10AM – 1pm to fill out new patient intake paperwork. Prospective patients must bring all of the following documents to intake appointments:
- US. issued birth certificate, or U.S. passport, or Voters ID card AND:
- Proof of current address (bill, rent receipt with your name on it)
- Florida driver’s license or state ID card
- Social security card
Once a patient completes paperwork they are placed on a wait list. When a dentist is available, patients are called in the order of intake on a first come, first serve basis.
Insurance Providers For The Indigent
Florida Medicaid
Medicaid is a medical assistance program that helps provide access to healthcare for families with low-income and individuals.
Florida Medicare
Medicare is a health insurance program for people age 65 or older. People younger than 65 can qualify for Medicare, too, including those with disabilities.

The program assists those that do not have sufficient health care coverage and/or financial resources for managing HIV disease or AIDS.
